My recommendations would be to book in advance. I've had problems before trying to buy tickets on the spot, it all depends on availability and the weather. If the weather is cloudy, as it often is around Lukla, there can be a huge bank up of people with prebooked tickets and then it is a nightmare trying to get out.
I've also had an occasion, some years ago, when I was very lucky and picked up a flight within half an hour of arriving at Lukla.
There can be price differences, dependant on whether you get a flight straight to Kathmandu or have to go via Lamidanda or somewhere else. I think the current price is around $90 US.
