I've cut and pasted a reply I gave earlier in the year viz:
"The route you describe is possible - theoretically. Practically however it is a non starter.
A side trip from Manang is a viable option although take camping gear to Tilicho and then return to Manang and continue the Circuit over the Thorung La.
Whilst there is a route over to Jomosom via Tilicho this is not recommended as the trail is faint, a definite job here for a good guide. More importantly, on the Jomoson side you would impinge on land used by the Nepalese Army School of Mountain Warfare. Having slogged over the chances are that you would be turned back to Manang by the army. In fact that is another definite!
As I say it is possible but requires careful planning and negotiation with the authorities to negate the 'turnaround'. They certainly aren't advertising this however."
This was the situation in April this year. It appears possible with a lot of prior planning and liaison but cannot be regarded as a last minute alternative.
