EBC: Living costs and flight booking

I and a friend of mine are planning a trek to the EBC in October.

1.How much cash should we bring on the trek, considering eating and sleeping in teahouses?

2. We are planning to fly out of Lukla, how much in advance should we book the flight? Does anyone have any tips or recommendations?
